Chronic care

A long-term condition is easier to live with when the same doctor follows it over time. We plan regular reviews, keep an eye on your results and help you register your condition with your medical aid so your medicine is covered.

What we help with

  • High blood pressure, diabetes and cholesterol
  • Asthma and COPD
  • Thyroid conditions
  • HIV care and PrEP
  • Epilepsy and long-term mental health conditions
  • Help registering your condition with your medical aid

How it works

  1. 1

    Long first visit

A 30 minute review of your history, medicine and recent results.

  2. 2

    A plan you agree with

Targets, medicine and the next review date, written down for you.

  3. 3

    Regular reviews

Usually every three to six months, with repeat scripts in between if you are stable.

Questions

Will my medical aid pay?

Most chronic conditions on the Chronic Disease List are prescribed minimum benefits, so your scheme must cover diagnosis, treatment and care once your condition is registered. We help with the forms. Your scheme may ask you to use its network and its medicine list.

Can I get my repeat script without a full visit?

Yes, if you are stable and seen for a full review in the last six months. Ask for a repeat prescription check.

Do you do blood tests here?

We do finger-prick tests on the spot. For other blood tests, we take the request form with you and a pathology lab nearby draws the blood. The lab bills you or your scheme directly.
