Privacy notice

How we look after your personal and health information, under the Protection of Personal Information Act 4 of 2013.

Who we are

Lantern Family Practice is the trading name of Dr N. Mokoena and Partners, a private medical practice at First floor, 112 Station Road, Observatory, Cape Town. We are the responsible party for the personal information we collect. Our information officer is Dr Naledi Mokoena, reachable at hello@lanternpractice.co.za.

What we collect

Your name, contact details, date of birth, ID or passport number, medical aid details, and the health information you and other healthcare providers share with us. Health information is special personal information under section 26 of the Protection of Personal Information Act (POPIA), and we only process it with your consent or where the law allows, such as to treat you.

Why we use it

To book and prepare for your visits, to treat you, to keep your medical record, to claim from your medical scheme, to send reminders you asked for, and to meet our legal duties under the Health Professions Act, the National Health Act and tax law.

Who we share it with

Only those who need it to care for you or pay for your care: your medical scheme, laboratories, pharmacies, specialists and hospitals we refer you to, and our practice software provider under a written agreement. We never sell your information or use it for marketing without your separate consent.

How long we keep it

Medical records are kept for at least six years after your last visit, and longer for children (until they turn 21) and in other cases the HPCSA requires.

How we protect it

Records are kept in access-controlled practice software with backups, staff sign confidentiality agreements, and only the people caring for you can open your file. If we ever learn of a breach, we will tell you and the Information Regulator as POPIA requires.

Your rights

You may ask what information we hold about you, ask for a copy of your record, ask us to correct it, and withdraw consent for anything that relies on consent. Email us to make a request. If you are not happy with how we handle your information, you can complain to the Information Regulator at inforeg.org.za.
